How the counts reconcile
The IPPS tables and the CMS code-file addenda agree once two details are handled. Table 6A lists 190 new diagnosis codes and the ICD-10-CM addenda add 190, the same codes; a parser that expects a digit in the second character finds only 185, because the new category QA1 (QA1.71, QA1.790, QA1.791, QA1.792, QA1.798) is the first in ICD-10-CM with a letter in that position. Table 6C deletes 30 diagnosis codes, matching the 30 deletions in the addenda. On the procedure side, Table 6B counts 181 codes because it includes the 80 codes implemented on April 1, 2026; the October addenda add the remaining 101, the same codes. Table 6D deletes 40 codes, 2 of them already invalid from April 1, against 38 in the October addenda. 100 of the new procedure codes carry the Table 6B asterisk: they were discussed at the spring 2026 ICD-10 Coordination and Maintenance Committee meeting and finalized too late for the proposed rule.
New ICD-10-CM diagnosis codes (190)
Table 6A gives each new code its CC level as a secondary diagnosis and the MDC and MS-DRGs it groups to as a principal diagnosis. The HCC column names the V28 category of the earlier code the FY2027 conversion table links to each new code; CMS has not mapped the new codes themselves.

Present-on-admission exempt list
Codes on the POA-exempt list take no present-on-admission indicator on the inpatient claim; the field is left blank. Every other diagnosis needs Y, N, U or W, and the indicator decides whether a hospital-acquired condition can lower the MS-DRG. FY2027 adds 62 codes to the exempt list and removes 12; the removed codes below need an indicator on discharges from October 1, 2026.

HCC overlay: new codes waiting for a CMS mapping
The PY2027 initial ICD-10-CM mapping that CMS uses for Medicare Advantage risk adjustment only includes codes valid in FY2025 and FY2026, so none of the 190 new codes maps to a V28 category yet. For 45 of them the conversion table names an earlier code that maps to a V28 category, listed below; CMS decides each new code's category when it publishes a mapping for FY2027 codes, so treat the overlay as pending. How quickly contractor articles caught up
Medicare Administrative Contractors list covered diagnosis codes in billing and coding articles, and each article has to be revised when codes are added or deleted. In the Medicare Coverage Database export whose update period ends October 5, 2026, 80 of the 190 new codes already appear in 79 current articles, while 22 of the 30 deleted codes are still listed in 27. A deleted code in an article is not an error by itself, since it still applies to earlier dates of service, but a new code missing from an article that covers its predecessor can deny until the contractor updates it. The LCD list links each policy to its articles.
Method and reuse
The tables are parsed from the FY2027 final rule Table 6 files (6A-6J, not reissued by correction notice CMS-1849-CN3) with their markers and continuation rows, checked against the ICD-10-CM and ICD-10-PCS addenda, and joined with the FY2027 conversion table, the POA-exempt lists, the PY2027 initial CMS-HCC mapping and the current Medicare Coverage Database export. Frequently asked questions
How many ICD-10-CM codes changed for FY2027?
190 diagnosis codes were added, 30 deleted and 4 retitled, effective October 1, 2026 (IPPS Tables 6A, 6C and 6E, matching the CMS code-file addenda). 3 of the new codes are MCCs and 56 are CCs.
How many ICD-10-PCS codes are new for FY2027?
Table 6B lists 181 new procedure codes: 101 take effect October 1, 2026 and 80 were implemented on April 1, 2026. 83 are operating room procedures and 32 more are non-O.R. procedures that still affect the MS-DRG. 40 codes were deleted (38 on October 1, 2 on April 1).
Do the new FY2027 codes map to an HCC?
Not yet. The PY2027 initial CMS-HCC mapping only covers codes valid in FY2025 and FY2026. 45 new codes replace a code that maps to a V28 category, so the family is known, but CMS assigns the new codes only in a later mapping release.
Which CC and MCC lists changed?
Table 6I.1 adds 3 codes to the MCC list, Table 6J.1 adds 56 to the CC list and Table 6J.2 removes 21 from it. The CC exclusion list gained 14,334 principal-secondary pairs and lost 3,206.
