Every active Local Coverage Determination in the Medicare Coverage Database, rebuilt from the September 24, 2026 export: 862 policies from 7 A/B contractors and the DME and home health contractors, 760 of them with a billing and coding article that lists covered ICD-10-CM codes. 169 titles are published by more than one contractor, and each of those has a comparison page that lines the versions up by state, revision date and diagnosis-list size. A Local Coverage Determination applies only inside the jurisdiction of the contractor that wrote it, so the first filter is geography and the second is the date of service. Open the policy for the contractor that processes the claim, read its covered indications and limitations, and then open the billing and coding article for the diagnosis and procedure codes the claims system tests; the LCD pages here show the article's covered-code count with a sample and link to the full list on cms.gov. Where a title appears at several contractors, the comparison page shows how far the versions have drifted, which matters for groups that bill across state lines. How many Medicare LCDs are there?

The Medicare Coverage Database export of September 24, 2026 carries 862 active Local Coverage Determinations across the A/B, DME and home health contractors, plus proposed policies not yet in force. Many are the same title written by several contractors, which is why this hub groups them into topics.

What is the difference between an LCD and its article?

The LCD states the clinical indications, limitations and documentation requirements. Since January 2019 the ICD-10-CM, CPT and HCPCS codes live in a companion Billing and Coding Article, which is what contractors load as the claims edit. Each LCD page here shows both.

Why do two LCDs with the same title have different codes?

Each contractor maintains its own policy for its jurisdiction. Policies are often copied from one contractor to another and then revised separately, so the covered diagnosis lists drift. The topic comparison pages line the versions up by state, revision date and list size.

Which LCD applies to my state?

The one written by the contractor that processes the claim: the A/B MAC for the state where the service is furnished for professional and institutional claims, or the DME MAC region for equipment. The contractor hubs list every active policy per jurisdiction.
