How the digest is built
The local sections come from the Medicare Coverage Database export itself: each LCD and article record carries the time it was last updated, and the export's update-period table gives the weekly windows, so a week lists every final and proposed LCD and every article whose current version was updated inside it, plus the documents retired in it. It reproduces the local documents the CMS what's-new report lists for the same window and, like that report, leaves out draft articles, which accompany a proposed LCD that is listed itself. The national items are those of the CMS what's-new report for NCDs and coverage analyses, matched to the week by their update date, and the open analyses come from the CMS national coverage analysis report, one row per analysis.
After each Thursday release the digest adds the new week and keeps the earlier ones unchanged, which is what makes the archive a record rather than a recomputation. How often does the Medicare Coverage Database change?
Every week. CMS records changes in weekly update periods and posts the downloadable export on Thursdays; each section on this page is one update period, dated by the release that carried it. Most weeks bring revised billing and coding articles; new, retired and proposed LCDs arrive in smaller numbers, often several contractors at once.
Why is a revision listed with an effective date after the week?
Contractors publish many revisions ahead of the date they apply, for example before an October 1 or January 1 code update or at the end of an LCD's notice period. The record is in the database now, but claims for earlier dates of service are still judged under the version in force on that date.
What is an open national coverage analysis?
A national coverage analysis is the CMS review behind a new or reconsidered National Coverage Determination. While it is open, the tracking sheet and, later, the proposed decision memo are posted on the database; a status of open for public comment means CMS is taking comments on it. The table lists each open analysis once, with the NCD page it relates to where there is one.
