How the CMS reference pages are built

This page documents the sources, computations, licensing rules and review process behind the MS-DRG, HCPCS drug code and Medicare coverage reference families, so that any number can be traced to the CMS publication it came from.

Data effective

Pipeline

CMS files are downloaded from cms.gov into a versioned mirror, each recorded with its URL, download date and SHA-256. A projection step parses the files, joins them by code or document identifier, computes the derived figures described below and writes one JSON record per entity. Pages render only from those records; nothing is fetched from CMS at request time. The projection is rebuilt whenever a source file changes, and the build fails if any CPT code carries a descriptor.

ASP changeOctober 2026 payment limit minus the July 2026 limit for the same HCPCS code, as a percentage of the July limit.
NCCI exposureCounts of active practitioner PTP pairs (deletion date blank) where the code is column 1 or column 2; modifier-allowed share is the fraction of those pairs with modifier indicator 1; partner lists are the most frequent counterpart codes. Pairs themselves are not published.
Richness and index statusNumber of joined datasets present (MUE, PTP, ASP, coverage, OPPS, add-on). Codes with a public-domain descriptor and at least three joins are indexable; sibling pages whose prose is too similar are kept noindex.
Lab NCD code countsRows in the January 2026 laboratory NCD spreadsheet grouped by NCD number; resolution code 1 counts as covered and 2 as non-covered; descriptions come from the FY2027 ICD-10-CM code file.

Licensing rules

HCPCS Level II, ICD-10-CM and ICD-10-PCS descriptors, MS-DRG titles, MUE values, ASP limits and CMS policy text are public domain and are shown in full. CPT codes are copyright AMA: the pages show the code number only, never a descriptor, short description or RVU, and every CPT number is flagged in the underlying data. NCCI PTP files were obtained under the CMS AMA click-through for internal use; the pages publish aggregate counts per code, not the pair tables. Where CDT or UB-04 codes appear in coverage articles, the ADA and AHA notices apply.

Review and freshness

Each page carries the QuickIntell RCM Editorial Team byline and the effective date of the data it was built from. Summaries and FAQs are generated by fixed rules from the page's own figures rather than by a language model, so a reviewer can verify each sentence against the table it describes. Pages show the live CMS release and the next expected release; a page whose data lags two releases is withdrawn from the index until rebuilt. Corrections can be reported through the contact page.

Frequently asked questions

Where does the data come from?

Every figure is taken from a published CMS file: IPPS final rule tables, the MS-DRG Definitions Manual, the HCPCS quarterly file, ASP payment limit and NDC crosswalk files, NCCI MUE and add-on tables, the NCCI PTP files (aggregated), OPPS Addendum B and the Medicare Coverage Database. The exact file names, versions, effective dates and SHA-256 hashes are listed below and on every page.

Why are CPT codes shown without descriptions?

CPT descriptors are copyrighted by the American Medical Association and QuickIntell does not hold a distribution license for public display. CPT codes therefore appear as bare numbers and are flagged as licensed; HCPCS Level II and ICD-10 descriptors are public domain and are shown in full.

How often are the pages refreshed?

The projection is rebuilt on each CMS release: quarterly for NCCI, MUE, ASP, HCPCS and OPPS (January, April, July, October), annually for MS-DRG (October 1) and weekly for the Medicare Coverage Database. Each page shows the release it was built from and the next expected release.

Is the interpretation text generated by AI?

No. The summaries, FAQs and guidance sentences are produced by deterministic rules from the page's own data (for example the severity level, the weight change or the MUE adjudication indicator), reviewed by the QuickIntell RCM Editorial Team. They are not free-form model output.
