October 2026: 46 codes added, 33 removed
The FY2027 ICD-10-CM update took effect on October 1, 2026 and deleted codes that the lab lists carried, so CMS term-dated them on September 30, 2026 and listed the new, more specific codes in their place. 32 of the 33 removals are such deletions, and 46 of the 46 additions are the replacement codes the FY2027 conversion table lists for a code removed from the same NCD; C75.9 on NCD 190.18 was removed while the code itself remains valid in FY2027. A claim with a date of service on or after October 1 must carry the FY2027 code.

January 2026: 432 codes added, none removed
The January 2026 release only added codes. 331 were added as covered diagnoses across 18 NCDs, and 101 were added to NCD 190.15 (blood counts) as codes that do not support medical necessity; 190.15 works by exclusion, so those additions narrow what the blood-count NCD pays for. No code was removed and none changed its result.

What to check
Confirm that the claim scrubber and any order-entry diagnosis pick lists use the October 2026 lists for dates of service from October 1, 2026; a scrubber still on the January 2026 lists accepts deleted ICD-10-CM codes that the contractor now rejects. For outpatient laboratory orders likely to fail the edit, an advance beneficiary notice is the usual protection; the NCD pages explain the denial codes that follow a failed edit.

Method
Each release is read from the section 508 version of the CMS laboratory NCD ICD-10 spreadsheet: asterisks that mark changed cells are stripped, rows whose ICD-10 term date falls before the release's effective date are dropped, and the wildcard row for unlisted codes is set aside. Lists are compared per NCD by ICD-10-CM code and resolution. Descriptions come from the FY2027 ICD-10-CM files, and replacements of deleted codes from the FY2027 conversion table. The CSV holds every ICD-10-CM code added, removed or re-resolved on a laboratory NCD list from October 2025 to October 2026: 511 rows, one header row and the columns releases, ncd, ncd title, change, icd10 cm, fy2027 description, resolution, previous resolution, term date, fy2027 replaced by. Its first line is a comment that names each CMS source file with its SHA-256, the data date and the attribution line to keep when you reuse the table.

Did the lab NCD lists lose codes in January 2026?
No. The January 2026 release only added codes: 432 in total, 331 as covered and 101 as codes that do not support medical necessity. None were removed and no code changed its coverage result.
Why are there no April or July 2026 lists?
CMS posted no laboratory NCD code list for April or July 2026, so the January 2026 lists were the current ones until the October 2026 release.
How does a lab NCD list affect a claim?
Medicare contractors run the national edit automatically: a claim line for one of the NCD's procedure codes is paid only when the claim carries a diagnosis the list covers, and is denied when every diagnosis is non-covered or does not support medical necessity. A code removed from a covered list stops supporting payment for dates of service after its term date.
