CMS descriptor for modifier QK

Medical direction of two, three, or four concurrent anesthesia procedures involving qualified individuals

CMS HCPCS Level II file, October 2026 (short descriptor: Med dir 2-4 cncrnt anes proc)

When modifier QK is used

QK identifies an anesthesiologist's medical direction of two, three or four concurrent anesthesia procedures. Each medically directed case is paid at 50% of the allowance to the anesthesiologist, with the CRNA billing the other half with QX.

Basis: Pub. 100-04, chapter 12, section 50. Summary in our own words; check the payer's current policy before billing.

Denials when modifier QK is wrong or missing

A modifier that does not fit the procedure code comes back with claim adjustment reason code 4 (the procedure code is inconsistent with the modifier used, or a required modifier is missing). When a coverage article requires the modifier and it is absent, the line is usually returned as unprocessable with reason code 16 and a remark code naming the missing information, which is fixed with a corrected claim rather than an appeal. The denial code lookup explains each code.
