CMS descriptor for modifier GV

Attending physician not employed or paid under arrangement by the patient's hospice provider

CMS HCPCS Level II file, October 2026 (short descriptor: Attending phys not hospice)

When modifier GV is used

GV identifies a service by the hospice patient's attending physician or nurse practitioner who is not employed by, or paid under arrangement with, the hospice. Those professional services related to the terminal condition are billed to the Part B contractor with GV instead of through the hospice claim.

Basis: Pub. 100-04, chapter 11 (hospice). Summary in our own words; check the payer's current policy before billing.

Denials when modifier GV is wrong or missing

A modifier that does not fit the procedure code comes back with claim adjustment reason code 4 (the procedure code is inconsistent with the modifier used, or a required modifier is missing). When a coverage article requires the modifier and it is absent, the line is usually returned as unprocessable with reason code 16 and a remark code naming the missing information, which is fixed with a corrected claim rather than an appeal. The denial code lookup explains each code.
