CMS descriptor for modifier GE

This service has been performed by a resident without the presence of a teaching physician under the primary care exception

CMS HCPCS Level II file, October 2026 (short descriptor: Resident prim care exception)

When modifier GE is used

GE indicates a resident performed the service without the presence of a teaching physician under the primary care exception. The exception applies only to qualifying primary care centers and a limited set of lower-level visits; GC is used when the teaching physician was present for or performed the key portion of the service.

Basis: 42 CFR 415.174. Summary in our own words; check the payer's current policy before billing.

Denials when modifier GE is wrong or missing

A modifier that does not fit the procedure code comes back with claim adjustment reason code 4 (the procedure code is inconsistent with the modifier used, or a required modifier is missing). When a coverage article requires the modifier and it is absent, the line is usually returned as unprocessable with reason code 16 and a remark code naming the missing information, which is fixed with a corrected claim rather than an appeal. The denial code lookup explains each code.
