CMS descriptor for modifier FB

Item provided without cost to provider, supplier or practitioner, or full credit received for replaced device (examples, but not limited to, covered under warranty, replaced due to defect, free samples)

CMS HCPCS Level II file, October 2026 (short descriptor: Item provided without cost)

When modifier FB is used

FB identifies an item provided without cost to the provider or supplier, or one for which full credit was received for a replaced device. Ambulatory surgical centers use it on device-intensive procedures; hospital outpatient claims report device credits with value code FD instead.

Denials when modifier FB is wrong or missing

A modifier that does not fit the procedure code comes back with claim adjustment reason code 4 (the procedure code is inconsistent with the modifier used, or a required modifier is missing). When a coverage article requires the modifier and it is absent, the line is usually returned as unprocessable with reason code 16 and a remark code naming the missing information, which is fixed with a corrected claim rather than an appeal. The denial code lookup explains each code.
