Match the member's exact HAP product, state, network, and date of service before deciding whether authorization is required. A policy for one product line does not establish requirements for another.
Record the submission channel, reference number, requested service and codes, supporting-document status, and follow-up deadline. Retain the payer's current response with the account record.
