
The Cost of Manual Eligibility Verification: A Practical Model
Calculate eligibility verification costs from staff time, exceptions, transaction charges, and attributable rework without treating assumptions as industry benchmarks.
Eligibility verification cost is not just the price of an electronic transaction or the time spent on a payer call. A usable model includes preparation, interpretation, exceptions, documentation, and rework that can actually be traced to the verification process.
Start with your own operating data. This guide supplies a measurement method and an explicitly illustrative calculation. It does not claim a national average, a typical practice's savings, or measured QuickIntell customer results.
Decide what one verification includes
CMS describes the eligibility inquiry and response as an exchange of information about an enrollee's eligibility and coverage. It identifies ASC X12N 270/271 Version 5010 as the adopted non-pharmacy standard. That describes a transaction; it does not mean every administrative decision surrounding a visit is completed by one response. CMS eligibility transaction guidance.
For costing, define the workflow from the moment a scheduled encounter enters the verification queue to the point the result or unresolved exception is recorded. Decide how repeat checks are counted. Keep the number of patients, encounters, inquiries, and resolved cases separate: one patient may generate several transactions.
Specify which work is inside the model. Prior authorization, patient estimates, and coordination-of-benefits investigation may share information with eligibility, but their full costs should not automatically be assigned to eligibility verification.
Collect a small, representative activity log
Sample normal operating days across your payer and appointment mix. Include both routine responses and exceptions; a sample containing only completed cases will hide unresolved work. Avoid using a week with an unusual outage as the sole baseline, but retain it as a separate resilience scenario.
Your internal log can contain:

Use approved identifiers in a controlled system, not patient details in a public worksheet. If staff perform another task while waiting on a response, do not charge all elapsed time as active labor for both tasks.
Calculate direct operating cost
For each staff role, multiply active minutes by its fully loaded hourly cost and divide by 60. Add transaction charges and the share of directly attributable software and support expense. State how shared overhead is allocated, or report it separately when a defensible allocation is unavailable.
Use a consistent period:
Period cost = active staff labor + transaction charges + allocated software/support + attributable rework
Cost per completed verification = period cost / completed verifications
Also report cost per attempted verification and the unresolved count. A low cost per attempt can conceal an expensive queue of unfinished cases. Conversely, a complex referral population may require more legitimate work per completed case than a stable follow-up population.
An illustrative calculation, not a benchmark
Suppose an invented monthly scenario contains 2,000 completed verifications. Staff spend 6 active minutes per case at an assumed loaded cost of $30 per hour. Separately recorded rework totals 20 hours at the same rate, and directly allocated software and transaction charges total $400.

Every input here is hypothetical. The six minutes do not include the separately counted rework, and the calculation omits costs for which no assumption was supplied. Replace every input with measured local data before using the model for a decision. Unknowns should remain unknown, not inherit these example values.
Attribute downstream costs carefully
Review a sample of denials or corrections before classifying them as eligibility-related. Record the evidence for the cause and whether another process contributed. A denial arriving after an eligibility check does not, by timing alone, establish that verification caused it.
Keep rework labor, delayed cash, and unrecovered amounts separate. A claim that is paid later is not necessarily a permanent loss. If finance includes a financing cost for delayed collections, document the amount, delay, rate, and calculation period. Do not add the entire delayed balance again as a loss.
Patient inquiries can be another source of work. Count the relevant staff activity while protecting patient information. Avoid turning an assumed dissatisfaction rate into a monetary benefit without a measured connection.
Compare an automated workflow on the same basis
Include implementation, interface maintenance, staff review, failed transactions, downtime procedures, and contract fees. Electronic responses do not eliminate the need to handle ambiguous information or unavailable payer services.
Run the same definitions for baseline and pilot periods. Separate faster processing from reduced expense. If a team uses released time for other work, describe that as capacity gained unless payroll, overtime, or another expenditure actually changes.
Assess both the average and the distribution of effort. A solution that helps routine work but makes complex exceptions harder may move cost without reducing it. Review the unresolved queue and the time needed to reach the right person.
Turn the model into an operational decision
List the most expensive, frequent, or disruptive exception types and assign an owner to each. Some improvements may involve demographic collection, staff training, or workflow handoffs rather than new technology.
